As part of the financial aid process, you will need to gather and submit supporting documentation. See below for the documents you need for the FAFSA and the DC OneApp.
Documents for the FAFSA
To complete the FAFSA, you will need the following information:
- Your Social Security number, which can be found on your Social Security card, and your parents’ Social Security numbers, if you are a dependent student. If you do not have a Social Security number, go to www.ssa.gov or call 1-800-772-1213.
- Your driver’s license (if any).
- Your and your parents’ alien registration numbers or permanent residence cards (if you or your parents are not U.S. citizens).
- Your and your parents’ current bank statements.*
- Your and your parents’ current business and investment mortgage information; business and farm records; and stock, bond, and other investment records.*
- Your and your parents’ W-2 forms and other records of money earned.*
- Your (and your spouse´s, if you are married) and your parents’ most recent federal income tax return:*
- IRS 1040, 1040A, 1040 EZ,
- Foreign tax return, or
- Tax return for Puerto Rico, Guam, American Samoa, the U.S. Virgin Islands, the Marshall Islands, the Federal States of Micronesia, or Palau.
- Note: If you or our parents have not yet filed an income tax return for the current year, you can estimate taxes based on the previous year’s return or pay stubs when you submit the FAFSA. Once you or your parents file the current year’s return, however, you will need to go back and update your FAFSA online before June 10.
- Your and your parents’ untaxed income records for the previous 12 months:*
- Social Security,
- Temporary Assistance to Needy Families (TANF),
- Welfare, and
- Veteran's benefits.
DC OneApp Supporting Documents
The DC OneApp is an online application. But to complete your application package, you also must submit hard copy of the following documents. All of these documents must be submitted together, otherwise your application will not be processed.
Application Forms You must print out, complete, and submit hard copy with original signatures (dependent applicants must have parent/guardian signature) of the following with your supporting documentation:
Domicile Verification To establish that you are a DC resident, you must submit one of the following:
- Certified D-40 tax return;
- Certified D-40 extension plus a certified D-40 tax return from previous year(s);
- 12-month history of:
- Child support;
- Retirement/annuity,
- Social Security income,
- Temporary Assistance to Needy Families (TANF) or or DC food stamps/EBT card,
- Unemployment benefits,
- Worker's compensation disability;
- Veterans administration;
- Ward of the court letter; or
- Other (includes applicants with deceased parents — contact your assigned HEFS counselor at 202-724-2824).
Utility Bill You must submit one of the following (less than 45 days old, reflecting your or your parent/guardian's name and address):
- One utility bill (does not include cell phones),
- A bank statement,
- A mortgage statement,
- A pay stub, or
- A letter from your landlord on rental office letterhead stating that utilities are included in your rent.
Note: Ward of the court and DC adoption applicants who lost one or both parent/guardian as a result of the events of September 11, 2001, do not have to provide domicile verification or a current utility bill. Applicants who were not enrolled in school during the most recent calendar year may be required to submit certified D-40s or other domicile information for previous years. Citizenship Documentation Certain scholarships are available only to U.S. citizens and eligible noncitizens. If you are not a citizen, you must submit one of the following (if you or your parent is an eligible noncitizen, you must submit the information for both you and your parent) to determine your eligibility:
- Alien Registration Receipt Form I-151/551,
- Certificate of Citizenship/Naturalization,
- I-94 Arrival/Departure record or computer-generated I-94A,
- U.S. passport or passport from another country with a valid INS endorsement,
- Permanent Residence Card,
- Status letter from INS,
- U.S. certificate of birth abroad, or
- Other.
Student Aid Report After you submit the FAFSA, the U.S. Department of Education will send you a Student Aid Report (SAR).
- If you complete the FAFSA online, you will receive the SAR as a PDF; you must print out hard copy of the PDF and submit it with your other supporting documentation.
- If you fill out a print version of the FAFSA, you will receive a paper version of the SAR in the mail. Make a copy and submit the copy with your other supporting documentation (you should keep the original).
Proof of High School Graduation (for first-time applicants only) To prove that you have graduated high school — or are on track to do so this school year — you must submit one of the following:
Transcript (for renewal applicants only) - Unofficial College Transcript
Adoption Scholarship Documentation If you are applying for the DC Adoption Scholarship, you must submit the following:
- Adoption decree (must be adopted from DC Child and Family Services Agency on or after October 1, 2001) or
- Copy of parent/guardian death certificate (for students applying because they lost one or both parents as a result of the events of Sept. 11, 2001).
*You need to include your parents’ information if you are a dependent student. If you are not dependent for financial aid purposes, you need to include only your own information.
